3rd play through on hard and I've yet to recieve this achievement. The third play through was flawless for me, absolutely no deaths and I'm not talking about "I died and then pressed the shut down button really quick" I mean I didn't die a single god damn time and I STILL didn't get this achievement. It seems just having the vita-chambers ON is excuse enough for 2k to skimp out on programming and screw us out of our achievements.
Now before someone decides to point out the obvious and go "hurr turn off vita-chambers" allow me to tell you that I CAN'T, THAT option is out on vacation somewhere. ALSO, before someone decides to direct me to the existing thread on how to correct that problem, let me point out ANOTHER PROBLEM, the DLC page of Bioshock is BLANK. That's right, boys and girls; every time I go to click it, it just says "Loading Content" only to give me some annoying error screen about how the requested content cannot be located. SO, if there's anybody from 2k actually giving enough of a damn to read this, let me tell you. FIX THIS PROBLEM.

It did for me. They had that link option since last year or so, don't recall.

Xbox handles the server where the BioShock stuff is located, 2K deals with the licensing.
