The biggest factor in my opinion was the fact that a simple automated "Confirmation Email" was not even sent to those who completed the claim form.
Instead of having the generic autoreply email, or heaven forbid assigning a customer number, you are left with an unsure feeling about whether your claim really was received at all. Sure, we all know very few things get lost in electronic mail, but that confirmation would have helped a lot of people to feel better about getting their replacement Big Daddy, even if MUCH later than anticipated.
Surely if you have talented people who can write code for a site and forum, let alone a game, there is someone who could have scripted that and prevented a classy gesture to take care of your customers from slowly turning into more bad press?
