I don't know it's already been mentioned, but I've discovered that if you get the AI to surrender multiple cities to you, you can cycle through all of the cities by clicking "View City".
When you've seen all cities you can return to the map, and choose Annex/Raze/Puppet for the last one you've seen.
All the other gifted cities are then annexed without any happiness penalty.
If however you save this game, and reload it the happiness penalty will be applied for all annexed cities.
